Madelaine Cohen

Systems rarely fail loudly. More often, they fail quietly: through one overlooked complaint, one closed file and one deferred act of accountability at a time.

My lived experience has given me a close understanding of what can happen when professional, regulatory and legal systems lose sight of the people they were created to serve. It has also strengthened my belief that evidence, courage and constructive engagement can create meaningful positive change.

I speak publicly about institutional accountability, professional conduct, document concealment, limitation periods that prevent legitimate concerns from being properly examined, and miscarriages of justice – particularly within Australia.

This is not commentary from the sidelines. My perspective draws on more than three decades of coaching and mentoring experience, together with my own sustained engagement with regulators, advocacy agencies, the legal system, journalists and political leaders.

My work is focused on turning lived experience into insight, accountability into reform, and difficult encounters of systems abuse into opportunities to build something fairer, stronger and more responsive for others.

Public appetite for conversations about institutional accountability is high and rising. Audiences are no longer satisfied with surface-level coverage of misconduct. They want to understand how it happens, why it persists and what meaningful reform requires.

This is a conversation that benefits from a voice with both lived experience and professional credibility behind it.

This is for producers, event organisers, journalists and policy contacts building a conversation around accountability, reform or leadership.
